*We cannot guarantee our services are covered by your specific plan. It is the client's responsibility to confirm any client costs (copays/deductibles/coinsurance) prior to beginning services. All fees are due at the time of the service.

If you'd like to make sure our services are covered under your plan or see how much is the client's responsibility, please contact your insurance provider directly, or send us your insurance ID number and DOB to run a courtesy check. This option may take several weeks to complete depending on the insurance company's responsiveness.

NOTE: We do NOT accept any Federal Medicare plans. If your primary plan is a federal medicare plan, we cannot bill to a secondary plan without a waiver/agreement from medicare on file.
